To assist people disabled by disease or injury to reach their maximum potential for an independent and productive lifestyle.

SERVICES: Physical and rehabilitative medicine, consultation in medical and surgical specialties, rehabilitation nursing psychology, physical therapy, occupational therapy, speech therapy, audiology, recreational therapy, radiology, laboratory, respiratory therapy and social services. Most of these services are available in the in-patient and out-patient settings.

ELIGIBILITY: Individuals referred by a physician.

FEES: Based upon a case by case basis. In most cases, insurance coverage is applicable to all Lakeview Rehabilitation Hospital services and the hospital is certified for Medicare, Medicaid, and Worker’s Compensation.

SERVICES: Community based, multi-service provider for individuals with mental retardation developmental disabilities. Agency provides rehabilitation, vocational and residential services for children and adults.

ELIGIBILITY: Individuals must qualify for waiver services or be assessed for services.
