To assist people disabled by disease or injury to reach their maximum potential for an independent and productive lifestyle.
SERVICES: Physical and rehabilitative medicine, consultation in medical and surgical specialties, rehabilitation nursing psychology, physical therapy, occupational therapy, speech therapy, audiology, recreational therapy, radiology, laboratory, respiratory therapy and social services. Most of these services are available in the in-patient and out-patient settings.
ELIGIBILITY: Individuals referred by a physician.
FEES: Based upon a case by case basis. In most cases, insurance coverage is applicable to all Lakeview Rehabilitation Hospital services and the hospital is certified for Medicare, Medicaid, and Worker’s Compensation.

The Kentucky Department for Aging and Independent Living (DAIL) oversees and monitors the administration of programs and services at the local and regional level, establishes policy and advocates on behalf of Kentucky’s elders and people with disabilities. DAIL works to support
and enhance freedom of choice and independence.
In partnership with Kentucky’s fifteen (15) Area Agencies on Aging and community partners to provide needed services and programs, DAIL is focused on addressing issues and circumstances that stand in the way of elders and people with disabilities achieving the best possible quality of
life.

Provides fair, compassionate treatment for victims and witnesses of violent crimes to ensure they are not lost nor forgotten in the legal process. These crimes include murder, reckless homicide, manslaughter, sexual or physical abuse, spouse or child abuse, robbery, kidnapping, wanton endangerment, and residential burglary.
OFFICE HOURS: 8:00 AM – 4:30 PM EST (Monday-Friday)
SERVICES: Help victims understand their rights and keep informed of all court dates.
Accompany victims and witnesses to court. Provide emotional support, and help file forfinancial aid through the Crime Victim Compensation Board. Notify victims when criminals are released into custody. Inform victims and witnesses of all probation or parole hearings so they may have input to the process. Identify services needed by victims, witnesses, and their families and refer to appropriate agencies.
SERVICE AREA: Breckinridge, Grayson, and Meade counties.
ELIGIBILITY: Must be a victim or witness of a violent crime, or a family member or survivor of a victim or witness.
